Patient Rights
As a member of Savior IPA Physicians Network, you have the right to receive care that respects your dignity, privacy, and individual needs. These rights apply to all members and their legally authorized representatives.
You Have the Right To:
Exercise your rights without regard to gender, sexual orientation, cultural, economic, educational, or religious background
Receive clear, understandable information about our medical group, services, providers, and how care is delivered
Understand how to obtain care across different settings, including primary care, specialty care, behavioral health, hospital services, and services outside the IPA network when applicable
Be informed of emergency and non-emergency benefits, coverage, and potential out-of-pocket costs before receiving care
Receive instructions on accessing care after hours and emergency services, including when to call 911
Review and understand bills related to services you receive
Receive information on how to submit claims for covered services
Know the name and qualifications of the physician coordinating your care, as well as other providers involved in your treatment
Have access to your primary care physician or covering physician 24 hours a day, 7 days a week
Receive complete and understandable information about your diagnosis, treatment options, risks, and expected outcomes
Discuss all medically appropriate treatment options, regardless of cost or benefit coverage
Participate actively in healthcare decisions and refuse treatment when permitted by law
Be treated with courtesy, respect, and full consideration of your privacy
Receive confidential handling of all personal health information and medical records
Be informed of healthcare facility rules and expectations regarding member conduct
Express concerns, complaints, or recommendations by contacting Member Services
Be informed of complaint, grievance, and appeal processes
Receive notice and assistance if your primary care provider or practice location changes
Change your primary care physician by contacting Member Services through your health plan
Receive information on how new medical technologies are evaluated for coverage
Experience reasonable continuity of care and timely responses to questions, requests, and appeals
Receive instructions for ongoing care following visits, procedures, or hospitalizations
Access interpreter services at no cost if you have limited English proficiency
Request enrollment in or decline case management and disease management programs
Request criteria used for any denial of services issued by the medical group
Member Responsibilities
To support safe, effective, and coordinated care, members also have important responsibilities.
Members Are Responsible For:
Understanding their health plan benefits and coverage limitations
Providing accurate and complete health information to their providers
Arriving on time for appointments and notifying providers of cancellations or changes
Reporting changes in health status as instructed by healthcare providers
Informing providers if they do not understand medical instructions or information
Following agreed-upon treatment plans
Contacting their primary care physician for care needed after normal office hours
Treating healthcare providers, staff, and other members with respect
Obtaining required referrals for specialty care
Following procedures for routine, urgent, and emergency care access
Contacting Member Services or their health plan with questions or concerns
Respecting the rights, property, and environment of healthcare facilities and staff
These responsibilities also apply to individuals legally authorized to make healthcare decisions on behalf of a member.
